The Town of Wellfleet invites you to apply for grant funding to provide one or more of the following services to residents of the Town of Wellfleet in Fiscal 2024:

• Domestic violence and sexual assault services

• Homeless prevention

• Food Co-op and basic services

• Food Pantry and basic financial assistance

• Case Management & support services for chronically ill residents

• Adolescent Drop-In Services

• Infant/Toddler childcare services

• School-age after school services

• Counseling services

• Drop-in services for substance abuse recovery

• Primary and specialty medical care services

• Civil legal representation

• Alzheimer's and Dementia support services

If you would like to receive the RFP for any of the above listed services, please contact me by email and I will forward it to you. suzanne.thomasrawellfleet-ma.gov. The application form is attached to this email and if you need additional space, please use no more than one additional page to provide your additional information.
